POSITION SUMMARY

The Infection Prevention and Control LPN will follow the Joint Commission (TJC), Arizona Department of Health and Services (ADHS), Occupational Health and Safety Administration (OSHA), Centers for Disease Control & Prevention (CDC), and other regulatory agency requirements regarding employee health and infection Prevention and Control Practices. Additionally, the IC LPN will identify health hazards and risks in the workplace, assess health statuses, and promote good overall employee and patient health.

Qualifications

ESSENTIAL SKILLS AND EXPERIENCE:

  • Valid AZ LPN License i[AF1] n good standing is required.
  • Minimum of three (3) years’ experience in healthcare required.
  • Valid, unrestricted AZ Driver’s License required
  • Employee health/occupational health experience preferred.
  • Infection Prevention and Control Experience Preferred
  • Electronic Medical Record experience preferred; NextGen experience strongly preferred
  • Certification to perform cardiopulmonary Resuscitation for the Health Care Professional (CPR) and AED through courses that follow the guidelines from the American Heart Association and Red Cross (cognitive and skills evaluations)
  • Valid Level One Fingerprint Clearance Card issued by the Arizona Department of Public Safety for all specialty behavioral health locations or ability to obtain within 30 days of employment.
  • Knowledge of the following regulatory agencies is preferred:
    • Occupational Safety and Health Administration (OSHA)
    • The Joint Commission
    • Health Resources and Services Administration (HRSA)
  • Highly detail-oriented and capable of developing innovative solutions to complex issues using available and custom tools. 
  • An ability to work independently, exercise creativity, and maintain a positive attitude. 
  • Ability to handle multiple tasks simultaneously and prioritize tasks without compromising the quality of work.
  • Ability to maintain the confidentiality of employee health records. 
  • Comply with the organization’s immunization requirements.  
  • Competency in working with people of various cultures
  • Ability to perform a variety of assignments requiring considerable exercise of independent judgment

 

POSITION REPONSIBILITIES

Incident Management

  • Collaborate with Key stakeholders to:
  • Investigate patient communicable diseases reported and collaborate with applicable state and local health departments.
  • Determine root cause and contributing factors
  • Gather and analyze data trends and develop action plans to comply with state and federal regulations.

Compliance

  • Assist with ensuring compliance with regulatory requirements related to infection Prevention and Control:
    • Adherence to Infection Prevention and Control Policies and Procedures
  • Participate in multidisciplinary tracers and IC observations pertaining to:
    • Infection prevention and control
    • Instrument Reprocessing
    • High Level disinfection

Education and Training

  • Stay current with regulatory requirements.
  • Provide education and recommendation for employees on IC:
    • Best practices and evidence-based practices
    • Policies and procedures
    • Identified trends

Data Analysis

  • Collect and analyze data related to infection Prevention and Control
  • Create Analytic reports and present data weekly, monthly, and quarterly during meetings
  • Contribute to the Quality committee and Safety Committee preparation through collaboration with the Director of Quality, with the Chief Clinical Officer, Director of Quality, Senior Manager of Quality Assurance & Safety and Employee health/Infection Prevention and Control Manager.

Collaboration

  • Communicate with employees to promote a culture of safety
  • Collaborate with the employee health team on the annual Respiratory Vaccine Campaign
  • Collaborate with key stakeholders on the completion of quality methodologies tools developed.

Survey Readiness

  • Assist with Infection Prevention and control initiatives associated with regulatory agency surveys
  • Participate in multidisciplinary tracers and IC observations pertaining to:
    • Infection prevention and control
    • Instrument Reprocessing
    • High Level disinfection
  • If applicable, participate in workgroups to address or improve regulatory requirement initiatives
  • If applicable, develop and maintain robust documentation and reporting of the following Infection Prevention and Control Quality Methodologies:
    • Action plans
    • CQIs
    • RCA
    • Process Mapping
    • Other IC associated activities

Program/Plan Support

  • Assist the Employee health/Infection Prevention and Control Manager in the implementation and ongoing maintenance of the Infection Prevention and Control Program and Annual Infection Prevention and Control Plan. Collaborate on strategies to ensure compliance with best practices and foster a safe healthcare environment.

 

 

NONESSENTIAL SKILLS AND EXPERIENCE:

  • Two (2) or more years’ experience in a Community Health Center

Patient Centered Medical Home (PCMH) and Integrated Care Team (ICT)

Physical demands: While performing the duties of this job, the employee may be required to sit for long periods of time, is required to stand, walk, use hands to handle or feel objects, tools or controls; reach with hands and arms; climb steps/stairs; balance; stoop, kneel, crouch or crawl; talk or hear; smell; manage stress as it relates to essential job functions.  The employee must frequently lift and/or move up to 25 pounds without assistance and may occasionally be required to lift or move up to 50 pounds with assistance.  Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

 

Work environment: While performing the duties of this job, the employee is exposed to weather conditions prevalent at the time when traveling to various clinic sites.  The noise level in the work environment is usually moderate. The employee may be subject to health hazards (contagious diseases, blood borne pathogens, etc.) when working in the clinic area.
