POSITION SUMMARY

The Registered Nurse Supervisor is responsible for the overall leadership of clinical nursing services, and assumes leadership of nursing personnel (RN, MA) at designated site(s). They will clinically supervise Registered Nurses and Medical Assistants and/or Care Coordinators, as assigned, in delivery of clinical care. The Nurse Supervisor will act as liaison with accredited Nursing programs. In collaboration with the Nurse Manager and/or Director of Nursing & Quality Improvement, the Nurse Supervisor will play a vital role in the preparation for on-site clinical and operational audits, as well as the development and monitoring of quality improvement projects and clinical metrics. This position is essential to Adelante’s team-based primary care medical home model of care that supports coordinated, patient-centered, comprehensive health care to all.

Qualifications

ESSENTIAL SKILLS AND EXPERIENCE:

  • Bachelor’s Degree from accredited university or foreign equivalent.
  • Minimum of two (2) years increasingly responsible nursing management, experience preferably in outpatient setting.
  • Minimum of five (5) years experience in direct patient care, preferably in an ambulatory care setting;
  • Graduation from an accredited nursing program.
  • Licensed with AZ State Board of nursing; license in good standing
  • Demonstrated understanding of clinical methods and techniques used in record-keeping systems.
  • Advanced computer efficiency in Microsoft Office suite (Word, Excel, PowerPoint, Outlook) and experience in Patient Management System
  • Meet the organization immunization requirement
  • Excellent verbal and written communication skills, including strong organizational, detail and interpersonal skills;
  • Demonstrated ability to work effectively with men and women of diverse races, ethnicities, ages and sexual orientations in a multicultural environment.
  • Valid Arizona driver’s license
  • Certification to perform cardiopulmonary Resuscitation for the Health Care Professional (CPR) and AED through courses that follow the guidelines from the American Heart Association and Red Cross (cognitive and skills evaluations)
  • Valid Level One Fingerprint Clearance Card issued by the Arizona Department of Public Safety for all specialty behavioral health locations

POSITION REPONSIBILITIES/ESSENTIAL:

 

  • Staff Development and Management (50%)
    • Assist and support in all human resources functions for nursing and ancillary service including hiring, firing, and disciplinary action; maintains appropriate documentation and ensures compliance with all company policies and requirements;
    • Ensures that staffing levels and coverage are adequate to meet patient care and regulatory requirements;
    • Assumes managerial duties for performance evaluations and maintains systems to provide regular feedback and evaluation for employees;
    • Works closely with supervisors and other company managers to maintain positive patient care atmosphere in health centers;
    • Assist the Director of Nursing in developing and implementing staff training and development programs to meet continuing education and contractual requirements and to further knowledge of medical care and working in team environment;
  • Program Planning and Management (10%)
    • Assist the Director of Nursing to provide overall direction and guidance for nursing & health programs in health centers;
    • Works closely with Director of Nursing and other company leaders as appropriate, to ensure that nursing program and ancillary services meet patient care and regulatory requirements;
    • Oversees and controls supplies that directly impact patient care (e.g., ordering and charging of stock medications, medical supplies and patient care equipment and/or supplies);
  • Quality Management (15%)
    • Assist the Nurse Manager/Director of Nursing to provide Quality Management direction and oversight for nursing and ancillary activities in all Adelante Health Centers as appropriate;
    • Actively participates in Quality Management Committee (QMC) and ensures that regular monitors are conducted and reported on;
    • Leads and/or participates in various special projects teams to address areas of concern;
  • Clinical Compliance (15%)
    • Assures that nursing and ancillary staff practice within their individual scopes of practice.
    • Participates in Nursing Unit Level P&P Committee maintaining current P&P for nursing and lab programs and ensures that they comply with all regulatory and contract requirements;
    • Participates in or conducts regular meetings with staff to ensure full understanding and buy-in for all Policy & Procedure (P&P);
    • Participates as needed in all program, licensure and other external audits and in the preparation of Corrective Action Plans (CAP);
  • Miscellaneous (10%)
    • All other duties as assigned.
  • Performs triage calls using approved clinical decision support tools for the adult primary care, OB, and Peds populations.
  • Participates in team meetings, designated “Specialty Team” meetings, and quality and process improvement initiatives.
  • Helps facilitate health and disease patient education (including adults, peds, and OB populations) sessions.
  • Coordinates continuity of patient care with external healthcare organizations and facilities such as referrals to outside specialties and ongoing two-way communication between providers and care teams.
  • Develops relationships with external healthcare organizations and facilities in efforts to find additional resources.
  • Coordinates outreach efforts to high-risk patients and hospital discharge and admissions. Will collaborate with health plans, hospitals, and skilled nursing facilities for transitional needs.
  • Conducts comprehensive, preventative screening for patients and/or assists all support staff in daily patient interactions as needed.
  • Promotes clear communication amongst a care team and treating clinicians by ensuring awareness regarding patient care plans.
  • Facilitates patient care based upon approved standing orders and protocols.
  • Works collaboratively with the clinical team on data collection, health outcomes reporting, clinical audits, and programmatic evaluation.
  • Review and take action on care team tasks and inbox messages.
  • Review and interpret lab results.
  • Identify barriers to care and social determinates of health.
  • Prepare for and assist with patient exams in the clinic setting.
  • Medication Reconciliation.
  • Medication Refills.
  • Prior Authorizations.
  • Referrals.
  • DME.
  • Short Term Disability Support
  • Completion of Nurse Visits including but not limited to POCT items (flu, strep, UA, HCG testing)
  • Helps evaluate clinical care, utilization of resources, and development of new clinical tools, forms, and procedures.
  • Manages insulin titration for the diabetic population, which can include outbound calls to follow up.
  • Excellent customer service and communication skills
  • Performs assessment and delivers patient-specific education plan.

Work environment:   While performing the duties of this job, the employee is exposed to weather conditions prevalent at the time when traveling to various clinic sites.  The noise level in the work environment is usually moderate. The employee may be subject to health hazards (contagious diseases, blood borne pathogens, etc.) when working in the clinic area.
