Primarily responsible for promotion, training, and nutrition education for all Adelante clients not enrolled in the Special Supplemental Nutrition Program for Women; Infants, and Children (WIC). They will also work with patients to develop nutrition-based self-management goals to improve their health. Additionally, responsible for creating, implementing, and managing nutritional curriculum for group-based classes. Works with Community Health Educators to provide targeted focus to UDS quality improvement for BMI. Lastly, this nurse position will provide services at external community events, migrant-worker clinics on farms, and our Medical Mobile Unit as schedule allows. This position is essential to Adelantes team-based primary care medical home model of care that supports coordinated, patient-centered, comprehensive health care to all.

  • Four (4) year college degree in Dietetics or nutrition-related degree and preferred two (2) years experience in community or clinical nutrition education programs or completion of a dietetic internship with RD/RDN credentials
  • Must hold current credentials as a Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) with the Commission on Dietetic Registration
  • Certification to perform cardiopulmonary Resuscitation for the Health Care Professional (CPR) and AED through courses that follow the guidelines from the American Heart Association and Red Cross (cognitive and skills evaluations)
  • Valid Level One Fingerprint Clearance Card issued by the Arizona Department of Public Safety for all specialty behavioral health locations
  • Documented excellence in customer service with the ability to interact with and instruct employees and clients from beginner to advanced levels
  • Demonstrated competency in nutrition education methods and techniques
  • Working knowledge of current Microsoft Office applications and relevant program-related applications
  • Competency in working with people of various cultures
  • Evidence of valid Arizona drivers license and current auto insurance
  • Ability to maintain confidentiality
  • Prioritization and multi-task skills are required
  • Bilingual (English/Spanish) strongly preferred
  • Correctly collect specimens, perform testing, and processes as indicated.
  • Travel to multiple locations required

  • Provide nutrition education and health promotion to clients and others from the different grants/programs as well as preventative strategies to Community Health Team
  • Develop educational guidelines for nutrition and preventive health promotion services according to the local standard care plan
  • Educate Community Health staff on nutrition education guidelines
  • Certify participants according to program guidelines
  • Assist in the completion of appropriate reports as required
  • Other duties as may be required or assigned
  • Plans, implements, and participates in community outreach activities as schedule allows
  • Attends meetings, trainings, and conferences for continued professional development
  • Conducts quality assurance according to federal guidelines
  • Follow up all high-risk participants by reviewing reports and signing off within 30 days
  • Participates in Community Health team meetings, designated Specialty Team meetings, and quality improvement initiatives.
  • Coordinates continuity of patient care with external healthcare organizations and facilities such as referrals to outside specialties and ongoing two-way communication between providers.
  • Develops relationships with external healthcare organizations and facilities in efforts to find additional resources
  • Coordinates outreach efforts to high-risk patients and hospital discharge and admissions.
  • Helps to manage high-risk patient care, including managing patients with multiple co-morbidities or high risk for readmission to a hospital setting. Provides data entry/management.
  • Promotes clear communication among the care team and treating clinicians by ensuring awareness regarding patient care plans
  • Works collaboratively with the Community Health team in data collection, health outcomes reporting, clinical audits, and programmatic evaluation.
  • Helps evaluate clinical care, utilization of resources, and development of new clinical tools, forms, and procedures.
  • Uses excellent customer service and communication skills

Work environment: While performing the duties of this job, the employee is exposed to weather conditions prevalent at the time when traveling to various clinic sites. The noise level in the work environment is usually moderate. The employee may be subject to health hazards (contagious diseases, blood-borne pathogens, etc.) when working in the clinic area.
